Hlavn\u00edm alergenem je u ryb parvalbumin, protein, kter\u00fd tvo\u0159\u00ed hlavn\u00ed slo\u017eku b\u00edl\u00e9 svaloviny ryb. Nejl\u00e9pe tolerovateln\u00e9 ryby pat\u0159\u00ed do \u010deledi Scombroideae, kam spad\u00e1 nap\u0159. tu\u0148\u00e1k.<\/p>\n<p>Alergie na ryby se projev\u00ed jako \u00a0\u017ealude\u010dn\u00ed a st\u0159evn\u00ed obt\u00ed\u017ee, kop\u0159ivka, quinckeho ed\u00e9m, mo\u017en\u00e9 jsou i astmatick\u00e9 projevy, du\u0161nost \u00a0a v nejhor\u0161\u00edm p\u0159\u00edpad\u011b i anafylaktick\u00e1 reakce.<\/p>\n<p><!--more--><\/p>\n<p>&nbsp;<\/p>\n<h2>P\u0159\u00edznaky<\/h2>\n<p>Alergie na ryby se m\u016f\u017ee projevit v mnoha r\u016fzn\u00fdch podob\u00e1ch. V p\u0159\u00edpad\u011b alergie typu I propukaj\u00ed p\u0159\u00edznaky b\u011bhem n\u011bkolika minut od po\u017eit\u00ed ryby. V\u00fdjime\u010dn\u011b se mohou projevy alergie objevit a\u017e za 24 \u010di 48 hodin od po\u017eit\u00ed.<\/p>\n<h2>K\u016f\u017ee<\/h2>\n<p>Na k\u016f\u017ei se objevuj\u00ed zarudl\u00e1 m\u00edsta s pup\u00ednky (tzv. kop\u0159ivka), otoky rt\u016f \u010di obli\u010deje. Pozd\u011bj\u0161\u00ed ko\u017en\u00ed reakc\u00ed b\u00fdv\u00e1 zhor\u0161en\u00ed atopick\u00e9ho ekz\u00e9mu b\u011bhem n\u00e1sleduj\u00edc\u00edho dne.<\/p>\n<h2>Za\u017e\u00edvac\u00ed \u00fastroj\u00ed<\/h2>\n<p>I v za\u017e\u00edvac\u00edm syst\u00e9mu se mohou p\u0159\u00edznaky alergie projevit s r\u016fznou latenc\u00ed. Posti\u017een\u00ed m\u00edvaj\u00ed nevolnost, pr\u016fjem \u010di zvrac\u00ed. N\u011bkdy mohou m\u00edt tyto projevy i prudk\u00fd charakter, stolice mohou b\u00fdt s p\u0159\u00edm\u011bs\u00ed krve, m\u016f\u017ee doch\u00e1zet k dehydrataci.<\/p>\n<h2>Anafylaktick\u00e1 reakce<\/h2>\n<p>Anafylaktick\u00e1 reakce b\u00fdv\u00e1 v tomto p\u0159\u00edpad\u011b sp\u00ed\u0161e vz\u00e1cn\u00e1, o to v\u0161ak nebezpe\u010dn\u011bj\u0161\u00ed. Posti\u017een\u00fd jedinec se m\u016f\u017ee b\u011bhem n\u011bkolika minut za\u010d\u00edt dusit, doch\u00e1z\u00ed k selh\u00e1n\u00ed srde\u010dn\u00edho ob\u011bhu, ztr\u00e1t\u011b v\u011bdom\u00ed, \u00fatlumu dechu. V tomto p\u0159\u00edpad\u011b je naprostou nutnost\u00ed v\u010dasn\u00fd z\u00e1sah l\u00e9ka\u0159e.<\/p>\n<h2>Vyvolavatel\u00e9<\/h2>\n<p>Nej\u010dast\u011bji je alergie na ryby zprost\u0159edkov\u00e1na alergickou reakc\u00ed t\u011bla na parvalbumin. V\u00edce ne\u017e 95% alergik\u016f je na n\u011bj sensibilizov\u00e1no. Je hlavn\u00edm alergenem. Jedn\u00e1 se o protein, kter\u00fd tvo\u0159\u00ed b\u00edlou svalovinu t\u011bla ryb. Sekvence aminokyselin parvalbuminu je u jednotliv\u00fdch druh\u016f ryb velmi podobn\u00e1. Proto se lid\u00e9 s alergi\u00ed na ryby mus\u00ed vyh\u00fdbat v\u011bt\u0161in\u011b druh\u016f ryb.<\/p>\n<p>Parvalbumin je stabiln\u00ed molekula a nem\u011bn\u00ed svou strukturu ani p\u0159i vysok\u00e9 teplot\u011b. Proto doch\u00e1z\u00ed k alergick\u00fdm reakc\u00edm i po po\u017eit\u00ed ryb tepeln\u011b upraven\u00fdch.<\/p>\n<p>Alergick\u00e9 reakce vznikaj\u00ed stejn\u011b tak po sladkovodn\u00edch ryb\u00e1ch, jako po t\u011bch mo\u0159sk\u00fdch. N\u011bkte\u0159\u00ed alergici sn\u00e1\u0161ej\u00ed ryby s p\u0159evahou \u010derven\u00e9 svaloviny, jako je nap\u0159\u00edklad tu\u0148\u00e1k \u010di me\u010doun. Pod\u00edl b\u00edl\u00e9 svaloviny v t\u011ble t\u011bchto ryb je n\u00edzk\u00fd, a tak obsahuj\u00ed i m\u00e1lo \u00a0parvalbuminu.<\/p>\n<h2>Prahov\u00e1 d\u00e1vka<\/h2>\n<p>Jako prahov\u00e9 d\u00e1vky ryb vyvol\u00e1vaj\u00edc\u00edch alergickou reakci (potvrzenou DBPCFC) se uv\u00e1d\u00ed:<\/p>\n<p>\u2013 treska: 5 mg,<\/p>\n<p>\u2013 makrela: 500 mg,<\/p>\n<p>\u2013 herring: 5 mg,<\/p>\n<p>\u2013 plat\u00fdz: 6000 mg.<\/p>\n<h2><\/h2>\n<h2>L\u00e9\u010dba<\/h2>\n<p>Vyvarovat se alergen\u016f. Alergici na ryby se p\u0159i v\u00fdb\u011bru potravin mus\u00ed pe\u010dliv\u011b vyhnout ryb\u00e1m \u00a0a ryb\u00edm v\u00fdrobk\u016fm. T\u011b\u017ec\u00ed alergici se mus\u00ed vyhnout jak p\u0159\u00edm\u00e9 konzumaci ryb, tak i v\u00fdpar\u016fm z va\u0159en\u00ed ryb\u00edho pokrmu, stejn\u011b tak nap\u0159\u00edklad kontaminovan\u00e9mu n\u00e1dob\u00ed od ryb. Je v\u0161ak t\u0159eba m\u00edt na pam\u011bti, \u017ee ryby jsou d\u016fle\u017eit\u00fdm zdrojem jodu, b\u00edlkovin a omega-3-mastn\u00fdch kyselin. Tyto slo\u017eky mohou ve strav\u011b alergik\u016f chyb\u011bt. Je t\u0159eba na to myslet a dopl\u0148ovat je z jin\u00fdch zdroj\u016f. Nen\u00ed od v\u011bci se o tomto probl\u00e9mu pobavit s odborn\u00edky na v\u00fd\u017eivu.<\/p>\n<h2>L\u00e9ky<\/h2>\n<p>L\u00e9ky slou\u017e\u00ed k o\u0161et\u0159en\u00ed ji\u017e prob\u00edhaj\u00edc\u00ed alergick\u00e9 reakce. Pokud reaguje alergik v\u00e1\u017en\u00fdmi celkov\u00fdmi p\u0159\u00edznaky, mus\u00ed se ihned zavolat rychl\u00e1 z\u00e1chrann\u00e1 slu\u017eba. Pokud m\u00e1 pacient u sebe od l\u00e9ka\u0159e ji\u017e p\u0159edepsan\u00fd pohotovostn\u00ed bal\u00ed\u010dek l\u00e9k\u016f, je t\u0159eba je pou\u017e\u00edt. Alergici by m\u011bli o sv\u00e9 alergii a o pou\u017e\u00edv\u00e1n\u00ed z\u00e1chrann\u00fdch l\u00e9k\u016f \u00a0informovat sv\u00e9 okol\u00ed (kolegy v pr\u00e1ci, p\u0159\u00e1tele, u\u010ditele, rodinu).<\/p>\n<h2>V\u0161eobecn\u00e1 doporu\u010den\u00ed<\/h2>\n<ul>\n<li>Lid\u00e9 posti\u017een\u00ed alergi\u00ed na ryby \u010di alergici samotn\u00ed by m\u011bli db\u00e1t t\u011bchto doporu\u010den\u00ed:<\/li>\n<li>P\u0159i n\u00e1kupu potravin si v\u017edy v\u0161\u00edmejte pe\u010dliv\u011b jejich slo\u017een\u00ed, vyvarujte se t\u011bm, kter\u00e9 by mohly obsahovat ryby.<\/li>\n<li>Vyvarujte se krab\u00edch \u00a0ty\u010dinek (surumi), lososov\u00fdch n\u00e1hra\u017eek a ryb\u00edch v\u00fdrobk\u016f.<\/li>\n<li>Dejte pozor v restauraci, aby j\u00eddlo neobsahovalo pochutiny z ryb.
